What is Support Coordination?

Guide Dogs SA/NT clients can access Support Coordination through our partnership with Guide Dogs Victoria. Their experienced team has more than 10 years’ experience helping people confidently understand and use their NDIS plans.

A Support Coordinator’s role is to guide you through your NDIS plan, help you connect with the right services and supports, and empower you to feel confident managing your plan.

Support Coordination is funded by the NDIS, so if it is included in your plan, there is no extra cost to you.

Practical and personalised support

How can Support Coordination benefit you?

Support Coordination helps you get the most from your NDIS plan in a practical, personal and empowering way.

Your Support Coordinator will:

  • Take the time to get to know you and your goals
  • Help you understand your NDIS plan and use it effectively
  • Connect you with the right service providers
  • Assist with organising appointments, bookings and service agreements
  • Build your confidence and skills so you feel more in control of your supports
  • Help you explore community, mainstream and funded services that support your goals

As part of the Guide Dogs team, our Support Coordinators have specialised knowledge in supporting people with low vision and blindness, paired with the same care and commitment you can expect from all our services. You can communicate with them in a way that suits you by email, phone or video call.

If your goals extend beyond Guide Dogs services, your Support Coordinator will help you connect with the right supports.

For those new to the NDIS or managing an existing plan

Who can benefit from Support Coordination?

You may benefit from Support Coordination if you:

  • Would like help to read or understand your NDIS plan
  • Want support to put your plan into action
  • Need help coordinating multiple supports or providers
  • Would like guidance exploring new services or supports
  • Would appreciate a dedicated NDIS expert who understands low vision and blindness
  • Want to feel more confident navigating the NDIS.

Support Coordination can be helpful whether you are new to the NDIS or already have a plan.

Funding options

What funding is available?

You may be eligible or already receiving funding from a range of sources including, but not limited to:

  • NDIS
  • My Aged Care
  • Children and Students with Disability (Department for Education)
  • Department of Veterans’ Affair

Support for some of our services is also available through agreements with other support agencies, or through Guide Dogs SA/NT’s charitable programs.

You can also get started in other ways:

  • Ask your NDIS Planner or Local Area Coordinator to make a referral using our provider number, 4050000113.
  • If you are already receiving services or taking part in a program at Guide Dogs SA/NT, speak to us about making a referral on your behalf.
